Summary The objective of the Sankofa Assistance Plan is to give immediate financial assistance to the beneficiary when a death occurs to a member. The Sankofa Assistance Plan is voluntary and is open to any active member of Sankofa. Upon notification of a death in the membership, payment will be made to beneficiary for the benefit amount, up to $5,000. A notarized copy of the death certificate must be forward to the Sankofa Board of Directors as soon as possible. The beneficiary will receive one-half of the benefit when the death certificate is received, with the remaining installment sent within 30 days. Summary 1. Only current members (paid annual or quarterly membership) may enroll in the Sankofa Assistance Plan. There is no physical required for initial enrollment. To enroll, each member will pay a one-time $50 enrollment fee. 2. Spouses of Assistance Plan members may also join the plan for a one-time $50 enrollment fee, providing they are named the beneficiary of the member and the member is the named beneficiary of the spouse. 3. When a death of a member of the Assistance Plan occurs, each member shall pay the amount of $25. This should be received by the Sankofa Assistance Plan no later than 30 days following his/her notification. Members and spouses who are both enrolled in the plan must each pay the $25 amount. Members who fail to comply with the provision will forfeit his/her membership in the Sankofa Assistance Plan. Forfeiture of membership will mean a loss of all benefits to be derived from this plan. 4. The Board of Directors may accept or reject any new application for membership. 5. In the event of the death of a member, his/her spouse may remain in the Sankofa Assistance Plan provided he/she becomes a member of Sankofa and remains in good standing with the plan. 6. Member ship will automatically be cancelled in the event any members to renew the $100 annual, or $25 quarterly, membership fee in Sankofa, or fails to remit death benefits within the specified time frame. 7. Any active member of Sankofa who has withdrawn or been dropped from membership of the Sankofa Assistance Plan may apply for reinstatement if he/she does the following:
The Board of Directors may then reinstate the member after any and all other requests made by the Board have been fulfilled and accepted by a majority vote. Records A complete record of all Assistance Plan members and deaths of its members will be maintained. The membership will be notified when a death occurs. For information purposes, a complete list of the members' names will be available at the annual meeting of Sankofa. This Assistance Plan is a major project of Sankofa, a non-profit organization dedicated to the empowerment of the family, especially the African American family. We believe that thorough grounding in the Nguzo Saba (Seven Principles of Kwanzaa) will restore our families and thereby our communities back to wholeness. These funds will complement the generous individual private donations and continue to support scholarships and operations for Sankofa.
